Scarlet said day 4 will be quite eventful but it is also said that characters are being introduced and the milf arc "takes shape in the next week or two". Although I'm pretty unsure whether the weeks mention irl dev time or in-game story time is.
I'd be pretty much happy if the update is going to be as big as you paint it to be but I'm saying this again: Unless the dev comes around and announces the next update to be particularly big, be it by animations, scenes or word-wise, I would refrain from stirring up expectations which might just backfire.

The only thing being big in this case is the reveal of new characters. I'm happy and very interested to see scarlet's work and vision coming to fruition. There are various teases in the post of what might happen but as long as nothing concrete is being said I would simply wait. Scarlet delivered in most cases 'good' updates by providing quality animations, well-written text and a good chunk of content in general.

Yeah high expectations will just kill the experience for you and hurt the game in the long run. The update will definitely be a morning update so I'm not expecting anything too crazy but the dev always has something fun to show in his game so I'm not worried.
It seems some of the new characters have been shown in futa on futa or hidden as random npcs in the past. For example the f*cking THICK waitress from day 3. Lord have mercy she's 2x THICKER than Hana. And apparently Ms Johnson too but I couldn't find her so I quit searching for her. .

There's no point. It wasn't written for an incest patch to work. Milly's surname is Scarlet and Amelia, Sophie & Ollie's surname is Futabu, the dialogue explicitly mentions and talks about Milly being taken in by Amelia

As Rexermus said, Milly being adopted by the Futabus is part of the plot. There's no easy replace stepsister with sister string in a editor. To make an incest patch you need to do some serious rewrite.

I also agree with Rexermus. There's no point in doing it since the game literally says Milly is an orphan. And it seems the dev will go deeper into it in the future so an incest patch will eventually make even less sense.
The dev has already even confessed that Sophie was supposed to be Milly's half sibling but the game was written to exclude any incest from the first version due to patreon’s tyrannical rules randomly taking down games with incest, and so Milly has no blood relations with the Futabas.

My headcanon is that Milly's just one of many futa kids left around by her mom(daddy?) but she was particularly unlucky to lose them. Otherwise the game would have Milly start fucking and knocking up women at 12 years old with a futa parental figure lol

So is there any actually STORY content in the last couple of updates? All I'm getting is basically an ad telling me what Day 4 will entail and some still images (screenshots, I presume).

Bruh this game needs to have a <map style> on your choices and the animations that you have unlocked. Game is really good since choices and endings can change from beginning but its very annoying since if you miss one you miss like entire line of story. Again, something very good that many games don't have, but fuck me if it isnt annoying.
