Look in options under controls, there's one for masturbating. The bare hands masturbation is done if you have nothing in your character's hands, and no pants or underwear on. The dildo masturbation is similar, except you have a dildo in your character's hands.

I can't figure out how to do this with a dildo, when I press the button it just do thos with hand, maybe there are some special conditions there?
