I mean the fact that the characters look like they're wearing too much makeup and shitty dollar store costumes.
Their designs read less "goddess descended from the heavens" and more "truck stop lot lizard."

So, reading the changelog, do arachne and the mosquito have a combined attack where they both grapple at the same time AND are compatible with the other two ladies, or did the dev mean that the two are separately able to "combine" (aka attack) the other two ladies now?

Again, check the article. It is clearly specified there. If not, article from yesterday is super clear on that.
Generally though, I think Kairi explicitly stated long ago that two different enemies will most likely never interact with each other because of some technical limitations. I guess it was because of how animations are handled (one enemy can trigger two enemy animations, so IDs are probably very simplistic here).
Mosquittos are now compatible with Blanc and Noir.
Arachne has combo for Airi though not for the other two.

Changelog certainly makes it sound confusing. How do you get to the lake area again? Didn't it used to have a warp in the bird menu but can't seem to find it.

Just lose to them? It will trigger a cutscene animation (impossible to see otherwise) but only once you have substantial debt (like 1000+) and no money/valuable items to pay it back.
