No, you can just set up the clothes through mirrors

Actually when the game first released you did lose clothing at low health. It was removed early on though, per the patch notes.

During scenes you can press V or B to toggle through your current costume's bottom and top options respectively, keep in mind it takes your current outfit into account so if you're topless it won't add any clothes or it won't switch between different styles in the same costume.
