What zapallbugs said, the guide is your friend. I'll add that the access is also time-gated, you must have been on the island at least 45 days before the accident can happen (and have advanced the twins quest quite a lot).
i just read the guide about the doctor work in underground lab and i cant find anything , i just load and jog , load again and jog in beach like a crazy . thanks man for the detail info

Kind of hard to answer with out giving out spoilers. Yes, there is some breast growth. One of the complaints most often heard is that the girls have large breasts. And by large, I mean 2 or 3 cup sizes bigger than what I suppose is considered 'normal'.
If you like big boobs, you'll probably enjoy the game or at the very least the artwork. You should know within the first 10 minutes of game play. If you have downloaded the game, double check that you have also download the additional bugfix. It kind of easy to miss, but the link is on the first page of the thread.
My only other suggestions is to download and read the Companion Guide, also linked on the first page of the thread.

i really wish a game like this could have permanent be as a mechanic in it but sadly i don't think it would happen ever thanks to the fact you would have to redo every single art and scene with the changed size witch is fucking ridiculous work. I was thinking sorta like how Natasha has those scenes a unique character could be made were they have less scenes and etc but they are a unique character as their whole thing is permanent be. So, basically a character with a small number of scenes and whatever, maybe three, but has permanent be.
