That is a hard question. There are many different games with this content. You have games focused on corruption, romance, story-first, slow burn, fast burn, harem, and so on. There are several top games for each of them. I advise you to try one of each and find out which type you like most. You could also do what I did when I started. Go to the Latest Updates, filter the Incest tag and Completed, and/or sort by likes, views, and weighted rating. Choose the one you find interesting.

In any case, this is my personal recommendation:

I really want to play these but haven't been able to get them to run - even when using a locale emulator. I'm running windows through Parallels on an M1 mac if that makes any difference.

Anybody have any ideas what the issue might be?

System locale and other region settings to Japanese if you haven’t already.

The Visit is probably a good one for this. The MC has a girlfriend, but you can choose to pretty much ignore her if you want.

If you don't have a problem with Japanese VNs, I recommend Itsumama 2 and Musumama 3. Both are about the MC and his mother, no one else. I'd say they're pretty good games in terms of story, quite short tho.

I don't but these are pretty much straight forward rathen than building up.

If you are talking Top in terms of actual Patrons and money, then Summer Time Saga will probably be top dog for a long time. Not a lot of other porn games can boast making millions. Treasure of Nadia is also huge, though the incest is not a focus in that game at all.

Well that is completely subjective. Ask ten different people and you will get ten different answers.
My personal favorite incest games are, Dreams of Desire, The Visit, Flirty F, Hailey's Story, and FILF. Games that are complete anyway.

It's not clear cause it doesn't have incest. After doing a little search in the thread, it looks like the dev isn't interested in adding incest to it.

No incest in that one. Which is fine, as it has been stuck in rework, remaster, limbo, for the better part of 3 years and will probably never get finished. It's big because player can customize the MC. Which means bunch of different versions of any renders with the MC in them. Also, dozens of branching paths for the story.

lozolmedeh, I'm not a fan of "golden rain", but I came across this fantasy in "Blossom of Pleasure", and in some others, if I remember, I'll let you know.

I just started Freeloading Family and noticed that the original game doesn't have true incest. The sister is a step-sister. There's a fan mod that changes the story to fit true incest. I also searched the thread to see if the dev changed it to fit Patreon, but that wasn't the case. So, I believe that we should include the tag MOD in it.

Quiet week. There's not much new incest games to add on the list for now. This is the time when mainly updates to existing games come out.
