I would like to say this is one of the absolutely easiest things to make your own skins for.
Use my template... This is a 2K resolution variant. So it'll look good on 1080 or 4K screens, while maintaining a 40kb file size.

Open this in Photoshop or Gimp (whatever your png editor of choice is)
Now find your favorite set of nipples out there from any photo or video, screenshot it, paste into Photoshop, then cut out one nipple. [I suggest if you are using your favorite porn star or whatever, get a scene where she's riding cowgirl with her arms up, then pause and screenshot on the highest resolution you can get -- the more straight on the better]
Now hide that layer you just pasted your screenshot into. (leaving only the base template and your new nipple visible)
Resize it so it's a near perfect circle and place that one over one of the nipple circles in the template.
Now duplicate it, Edit>Transform>flip horizontally, then click the move tool and slide it over the other nipple using the left and right arrow keys. (this will keep it perfectly aligned to the other)
Reduce the opacity to about 65-85% for the layer, the higher the opacity the darker and more evident it will be.
Now make the base layer (template) invisible.
Now select save for web, then navigate to "root>userdata>overlays" and save as png24 with transparency.
Open the game and open the character maker.
Go to the "Body" tab and then navigate down to Mods>Skin Overlays
You can set it as a body overlay or underlay (I prefer underlay). Then select the nearest 3D nipple model shape you want and adjust accordingly in the Body>Nipples tab to make it shaped how you want.

This may cause a slight offset to the image, look at where the image is off center then resize it in Photoshop accordingly then reload the overlay. If it is oversized on the outside edges, drag the bounding box tool over both nipples, then click the move tool and drag it one pixel in from the left and one in from the right then export and reload in the editor. If they are both off to the left, or both to the right, drag over with the bounding box, click the move tool and move them the opposite direction with the arrow keys one pixel at a time, exporting and reloading the textures of course, until they are properly aligned.

Now go to body>nipples again select the color as a red on the far right slider (I usually use the top one for Caucasians, but you may want the bottom for African/Caribbean or maybe even more of an orange, then orange/yellow for Hispanic/Asian), then in the box, slide it far left and up and down in the grayscale area until the color is close to what you want. You can add color, but it will likely be better to set the base color in the Photoshop template to the original nipple you are replicating.
Then slide the shine around until it's where you want...
Done, you have a custom nipple. (it sounds long but only takes a few minutes)

NOTE: Those weird shadow lines in this screenshot are not on my screen, no idea what's up with that... LOL

The other route is far more complicated and involves writing a .unity3D file in Unity and work in Blender or a 3D modelling software equivalent plus creating your own normals, smoothness, height, and occlusion maps (--I suggest Bounding Box's Materialize for that if you're going this route -

--). If you have Unity and want to use this route, copy the files in the root>mods>sideloader modpack>Praline>[Praline] nip_illu_mix.zipmod and save them to a new location. Then navigate to root>[MODDING] Tools>ZipStudio v0.2.1 and launch zip studio, unzip the Praline Zipmod and drop those files into your Unity workspace and edit them there, then save, export, use the ZipStudio to wrap a new zipmod and drop it into your Sideloader Modpack folders.

NOTE: if you are looking for premade custom packs, good luck... You'll probably have some luck on the Japanese Booru pages, but not so much elsewhere.

Nah ,alreay try to find on every forum but only HS1 are available
You could use this,it's actually more ingame-likely
I'm just curious about the default one,hope someone will make one soon

Well, she is a doctor in the past ... (Angela Ziegler). So I think she needs a gown and underwear. Changing her clothes is not a problem :)
