If you mean early on, he has no sense of self. He is effectively a floating consciousness.
Later on he can choose a humanoid appearance to present himself as, which can be discarded in favor of another appearance if that pleases the woman he chose since at the end he only cares to please and feed off her.
Since the point is to please those women, appearing with such an outlandish look would make his task harder (Unless they were into that).

Not really... they are magical demons with the power to seduce and corrupt. Their appearance is meaningless, because their victims have no choice in the matter. They don't exist to please women, women exist to please them (at least if you ask them). But your world, your lore. Nothing wrong with making them your own. Sort of like the Dark Brotherhood porn version.
