I've seen this in some other games as well...I would venture to guess that it's some kind of problem with the 3D model(s) itself.

Usually caused by forgetting to adjust the nipple sliders (often reduced to avoid pokethrough then not adjusted back afterwards). I know some devs also reduce it (when naked) as they can appear like this often in real life but on a Daz model it just looks completely off.
I didn't mention this one specifically of Mercy, but I mentioned some others from part 1 and past epsiodes that have simialr problems. She said she has a theory on why its happening, cause she is not doing it on purpose, but will look more into when part 2 is done, since when I mentioned the nipple issue a month ago, she was too far along in development, to change much right now.
