You can either start at K4 and get promoted to K5 or just start K5 or K6 and when you get the scene where you massage Alessandra, choose to focus on her feet and agree with her.

Yes, there are considerable differences in the dialogues if you are comparing secret sub to sub or dom. You'll get specialized scenes while still getting the majority of the dom content.
