i know they removed the timer selection for side jobs, is there a way to get it back in in the newer versions? like a mod or something?

No, you can only change hair color and pube color from pube dlc which if you downloaded full game here, will be there. When you boot the game, within options there should be Cosmetics(I think) that has option to change hair color and pube tweaks.
Someone is developing a cosplay mod, but I am not sure if it will change her body sizes.

The weekly dev diaries show that there were no plans for floor 4 for game balancing reasons, considering it happens right before the final boss floor. From what I remember, he did not want anything on floor 4 since it would be kind of pointless.

I can understand Floor 4 not having any edicts but they could have given the infirmary a purpose. It could have been like Karynn going around and nursing the patients back to health or a POV thing like the gym.

The infirmary is used. If you lose the final fight, Karryn wakes up in the infirmary. So, however briefly, it does get used.

But why though...? That's some bad omen when one of the first things dev thinks about in remake is Karryn being fucked in nipples. A common thing with "fan" remakes is they like to add in their own content before having even having the og/base game finished.
Dev should be focusing on the other side jobs (which is a bigger appeal for the remake personally) rather than a more obscure fetish like nipple fucking.
It may have been asked before, however where is the main js file located/what its name. As I read you can change the lines Karryn says if you lower the slut level value

I just realized how little interaction with the other main characters like Tonkin and the doc there is outside of story progress.

It's not even that. She wakes up in the bed and can really only exit the room. I think you can talk to the guy in there, but it's not Carville, and IIRC he only says one line. It's been a while since I've seen it.

It was left in for some reason. I guess Rem didn't want to bother removing it completely and left it in coding. Originally it was supposed to have its own side job or way to make money off massages. But again, we see how that went with content removal.
