Load a save before the scene BJ with Camelia, after you pass that scene you should get a scene walking and getting the option to buy a guitar, buy the guitar and after scene with "Mom" you should get the new scene with Iris

The game probably just checks if a file with that name exists and then activates the incest code if it does find it - therefore the file doesn't need to contain anything, just be named correctly.
