You can find the axe in the security room (you need the jump boots to reach it)

When you enter the woods, look to the north, there should be a rope hanging down.

Andy will give you a hint where the first grapling hook is. You find it outside near the river wall, when you find it, the game says that you hit something hard, you need pick axe to get the grapling hook.

axe is in security , on the lockers (northeast), you need the jumping shoes, which are on the first floor (which need 7 radiators turned on)

You could have used the search function.... Hint, you need to go to the graveyard and look at the stones that have dates of death.
