Yea, none of the unique characters can get a boob job (yet?
) and only the feMC can change her hair, eyes, etc.The surgery / mirror were mainly intended for the random natives. For now at least.
So... how do I play? I start the game... I run around the island.... and I couldn't find anything for 30 minutes. Then I returned to spawn and got ganked by like 5 guys and me and the other NPC got killed in seconds. I saw a little crafting on the list and I made a couple of things, but I never found string, fabric or anything else that could give me weapons or armor. Is there any indication of where to go or what to do?

My advice would be to gain some levels, then go fight the big spider. The "Spider Storage" you get from it's bench can be placed around all sorts of crafting stations, and it's a massive headache saver.

I grabbed it from a shady pirate repository, praying that there won't be any viruses.
It's in the western part of island, in a cave above the dense forest.
Basically, just run north-west from the starting beach location.
the raid system simple when the raid start, they select target ( your native with blue circle on them ), if the target down, they capture them
some time later rescue mission will occur, timer 20 minute will start for the rescue, if the capture female or native girl and the timer lower 10 minute they will get rape ( but not instant, enemy will try knock down your native first), otherwise when timer hit 0 your native will get kill
As far as I know, the inactive MC or other NPCs won't be captured (They can be knocked out, but they'll get back up after a little while), but any of your captured natives is fair game. When you're being raided the one being targeted will have a circle around them. If the targeted native is KO'd you'll get a "quest" to rescue them from a nearby enemy camp. Failing to do so will have that native killed.

Oof. I found Bigfoot and got one shot cuz that's what the guide said... But I have no levels. Maybe I'll just have to use cheats from spawn and just be god. Cuz clearly I cannot survive lmao

Try to scout Native bases, the smaller they are the weaker their inhabitants, directly north of the spawning point and east close to the coast are 2 weak settlements where you can fight natives 1 on 1 for a few Levels, they also tend to have mirracle berries for exp in their camps
Find thin trees tor fiber which can be turned into ropes for the spear and you can deal with most natives on this side of the mountain range
Does the moral applies to all? I mean the moral of the MC, not the NPC, I was about to convert into cannibalism until I saw cannibalism decrease your moral, so I wonder if it's also affect NPC?

Guys one question, when you move base (blue flag), the other MC keeps going back to initial beach, is there any way to stop it?
The combat system here is similar to Dark Souls. In a sense that it depends almost entirely on your skill. Your stats don't actually do much.
And by cheating yourself million levels, you're just gonna break the game. Because each raid will spawn huge hordes of natives, scaling to your level.
My best advice is literally "Git Gud".
They start out as a ludicrously rare spawn, and increase in spawn chance as the raid level goes up. Putting down totems to increase number of raid members also helps.
