Always a shame when someone spends time and effort to make hot family members who are vital to the story, but it doesn't have incest just because of patreon.
There are several ways around it; let the player set the relationship (classic landlady and roommate as default) or a toggle switch in the menu, put their lewd scenes behind a unofficial patch, just don't rely on patreon but use subscribstar, itch, steam and other funding sources, all of which allow incest, or "separate" the game from the patreon (have you patreons be subscribing for art and images and the game is released on discord or something).
It also makes me worry, because the dev says there won't be NTR, but I've heard the old song-and-dance before and you can call me paranoid but it's happened so often. If a sex scene of the sister or mom or other family member shows up with someone else it's "NoT TeChNiCaLLy NTR," because they aren't love interests. After all, it's hard for me to imagine a dev of an adult game making attractive girls and not ever using them in sex scenes, it's why so many devs end up making "not technically," content with rejected girls, side girls, or non-love interest girls. You spent all this time and energy making good looking girls, why wouldn't you make sex scenes with them and want people to see them. But here, with no incest but attractive family members... well, it makes me hesitate because I've seen this kind of thing before.
Now, maybe I'm wrong, maybe the no incest is just to keep patreon from seeing it and hammering it but there are plans for a patch or the story will resolve it in some way to get around things. Not every game needs incest and while I play plenty of games without incest (as long as they are a harem) when you have a game with attractive family in it but it won't have incest, well, I will be waiting until we see where it goes because again, I've seen it happen before.
Good luck to the dev no matter what ends up happening, and again if I'm wrong then I'll be pleasantly surprised.
