I finally have time to start this and immediately come to a roadblock.

as a first time player, this is not very intuitive. 'default' obvious is obvious, but the other two need better description than names of people you have never met.

While yes, I agree. But you had to download the patches to get those extra options. Which suggests you know what they are.

I'm limited in how I hook into the existing game to display the patch options. With previous patches, you didn't get a choice, and it loaded straight into which one you had installed.

How would you prefer to see the options?

there is a tiny bit of info in a readme in the patch, maybe expanding that to a short description to differentiate would be enough? deep-diving the thread can reveal that the Wilson patch is the "standard" uncensor patch, but without context its just a bunch of names.

again from searching the thread, it seems the Thomas option is additional/separate routes? which probably would not be best for a first experience.

Less is more when it comes to spoilers, but I can only suggest the tiniest bit of context alongside or instead-of the names.

edit: long story short, I just wasnt expecting multiple options from a single patch.

Understandable. It's actually two patches, not one. Either patch is fine for a first player; it just depends on who you prefer as the Mom and sister(s)

The original EyePatch (as in IPatch) would probably be considered the "standard" uncensored patch. I added the MPatch because Maria is hot, and many players, myself included, like the idea of her being the Mom.

Does the following description clarify it? If so, I will include it in the patch readme file.

The base game does not have lewd content with the Mom, and the MC, Max, has no siblings. There is no landlady or roommates to change relationships into mom/sister.
Max lives in a typical suburban neighborhood. There is lewd content with the neighbors on either side of Max's house.
The two patches included moves Max next door, either in the Thomas or Wilson families, giving a mom and sister(s) with lewd content.

EyePatch v6.1.1 "Wilson Family: Victoria, Catherine."
This changes Victoria into the Mom and Catherine into the older sister. It also makes Cynthia the aunt, with Kimberley (and Ralph) as cousins.

MPatch v6.1 "Thomas Family: Maria, Karla, Caprice."
This changes Maria into the Mom, with Karla as the older (bitchy) sister and Caprice as Max's twin sister.

If you disable NTR, will it change the images and lines? Or will we not see the scene but NTR will happen?
From the in-game help.

Netorare:
Enables content where a love interest may cheat on the player with another male character.
When disabled, a love interest will not cheat with another male character. However, they may still end a relationship or invoke other consequences.
Note: this only affects love interests and other male characters.

Netorase:
Enables content where the player can choose to share a love interest with another male character.
When disabled, the player cannot share a love interest with another male character.
Note: this only affects love interests and other male characters.
They are two separate settings. You can disable only one or both.
When disabled, the Netorare/Netorase does not happen with another male character.

The relationship points you build up with the male characters are only to determine their attitude towards you.
Ralph, for instance, will have your back if you have a high affinity with him. Otherwise, he'll be more likely to drop you in it.

the characters arent ugly, they unique and thats nice to see around so many Visual Novels.
Also the Mothers are Milf of course they should look a bit more mature. Humor is good too. Looking forward to more Updates.
