I just finished a long, slow play of this game, where I really took the time to read all the dialog.
In non-spoilers, the story is deeper than it first appears (I'll need to do this slow play with VC and Ren now) and there's a lot of character introductions you see in future games I completely missed a few years ago.
In spoiler town:
This game / world continues to fascinate me years later. I've self-inserted into the story in my head many times, usually in a manner to stop some of the more brutal parts.
I've written so much more, but this is probably long enough for now, and I doubt anyone cares this much about the world-building and story of what's supposed to be a porn game, except me. I'm left with thus: despite the brutality, it's my favorite femdom story and world. I hope for many more years (decades?) of it, and I'd recommend this game to any femdom fan (with the disclaimer, that if extreme femdom isn't for you, just play chapter 1).

When do we get to see Maria have sex with another guy? I'm at the start of chapter 4 and it hasn't happened yet
She has two encounters with other men if you make the right choices. The guy in the pink suit (I forget his name, it's been so long since I played it) and then later her friend sets her up with a slave with a big cock.

Yeah she's just had sex with Gregor in chapter 4, took a long time to get to it though
I didn't know Gregor. I replayed the game. The new game (The Renaissance) should also include cuckold theme.

There's only one sort of 'good' ending, and I have to pull a muscle reaching to justify it.
