A more clear summary from some one

Friend took heroines v-card years ago during a "folly of youth" (he alludes to this during one of the earlier sex scenes), MC thanks her for confessing about it after. This little plot tidbit seems to be a recycle from a few of their other games lol

Friend gets fucking PISSED and feels betrayed to the extreme when the MC accuses him of not actually dying of Mystery Disease X, which is his motivation for being the stereotypical evil NTR man in the middle of the game. In the worst ending (where the MC insists he's a fucking faker leading the heroine to huff off to be friend's live-in temp wife) he records a final grudge video to give to the MC after he dies where he fucks the heroine hard (saying he's going to overwrite everything about the MC with his coom lmao) and makes her swear to never be with him. She follows the friends instructions, telling the MC they can only ever hope to get back together after "a very, very long time" while handing him the aforementioned video and fucking off with a grin on her face. MC despairs that he didn't listen to his fiance and make amends with their friend.

At this point MC, at heroine's behest, goes to the friend but has to make sure not to do anything and see (proof of friend's sickness). Friend is having rough sex with heroine and MC is furious inside. While friend angrily ejaculates inside her while purposely ignoring MC, heroine tells MC to watch...and friend gets shallow breathing and can't breathe. MC rushes over to him with deep concern, apologizing for not believing him and heroine and offering to get him to the hospital because there's clearly something wrong. Heroine must be right that the sickness was also influencing his mental state and actions! MC princess carries him into bed and the two reconcile with the friend seeming to partially realize that his actions are kind of a stretch to forgive.

Around this point is where the "please have my child" thing comes up. Heroine actually initially declines and says he should meet someone special and have it with her, to which the friend says that he very likely doesn't have the time to forge those kinds of bonds with a stranger and that he's a troublesome burden enough as is. It has to be with someone he's close to and trusts. Ever the advocate for friend, Heroine is okay with having his kid but both her and the friend realize that it's asking a LOT from MC. This is where heroine goes even more stupid than usual and tells MC to break up with her. MC just goes lol no so she huffs off for two days just to record a no birth control nakadashi sex video with friend to show MC where she's really hamming up some of her reactions (and acting guilty in between). She presents it and says there, proof of infidelity and proof that she belongs to THAT man. MC points out the suspicious parts in the video and says she's forcing herself (as usual) for the friend but decides to give her an answer to the break up question.

Neutral ending is MC agreeing to the heroine's suggestion that they break up, she insists that because of her selfishness and betrayal of him that she deserves the hardships of being a single mother despite his insistence of offering financial support. MC meets with her and congratulates her on being pregnant, but can't bring himself to say anything towards his friend. In this ending she sounds pretty dead inside.

Good ending is the MC refusing to break up with his fiance and agreeing to her (and their friend's) idea for her to get pregnant with his child so he has something left behind in this world. He'll raise the kid, regardless of them not being a blood relative. Friend actually has a bro moment here where he demands that instead of just him, he AND the MC take turns ejaculating into the heroine so the kid could be either of theirs but the wife only wants to do it with him (and the MC, after some internal waffling, agrees). Heroine gives birth to friend's kid and he dies shortly after while thanking the both of them dearly. Fast forward to the near future, MC is having sex with the heroine and she's pregnant with their own kid who friend's kid will be an older sibling to. Probably one of the happier endings in these games.

Thanks. I'm gonna play the game, cause it look interesting now.
But honestly, if a friend was mortally ill, and asked for sex... i'd hire an escort for him, you know?

Man, this title is such a lie...

McCuck-kun, your girlfriend can't be stolen if you gift-wrap her and present her to your friend on a silver platter with a smile on your face. Seriously... The friend doesn't initiate anything at all... and only starts acting iffy with the gf when his illness starts going out of control. However, nothing would have happened if McCuck-kun didn't convince the girl in the first place to help him deal with his lust and then even have sex with him... The guy was perfectly reasonable up until he started losing his sanity, and odds are that said sanity loss would have been completely different if he wasn't having sex with her all the time.

However, the weakest point in the writing is the fact that the couple didn't ask for details about the illness, about the examinations that were made. They didn't make any effort to help their friend get a second or even third opinion, which is important when something genuinely life threatening pops up on the table. I've never been sick of anything super serious myself, but certain family members of mine had friends that ended up with stuff (cancer) and they did more than just take things at face value. Even when something life-threatening is found, a different doctor can give you different treatment options that might make all the difference.

Of course, if they did things logically, then of course we wouldn't have had the silly drama about the illness being real or not.

Beyond that, the girl definitely has some screws loose in her head too...

All in all, for me, this feels like one of the circle's weaker releases. It's like they wanted to have NTR themes but didn't want to commit to them properly, while also trying to avoid the netorase cliches from their other lines of titles. I dunno, it was all very weird, and even the so bad endings felt more stupid than anything else.
