Since it says "extract the game before playing" I would guess you're opening the file from the archive. You need to first unpack it with 7zip or a similar program.

One of the pills removes a negative trait.
1. Close the game.
Right click on the RAR, select "7-Zip" from the sub-menu then click the "Extract" button. Type in the drive letter you want the files to be extracted to in the "Copy to:" box. Like C:\ or D:\ and 7-Zip will do the rest..
Question, did you uncheck the new staff shift function?
Not yet and only because I don't know what all of the traits are or what they do. But that's not a bad idea. I could add something like that in the next guide. But it'd be a partial list.
