It has been a while, but if I remember correctly, to get the NTR ending you must reach the endgame without any main lover whatsoever. So even if you break up with Carol, if you keep for example Naomi as lover you should be able to see the ending with Carol just as a stepdaughter but on "good terms" without any NTR involved.
