There's instructions on the files on how to switch language to English. Look at trasnlation_readme

There is new game called NTRAholic DX which will be on steam. With more content and uncensored. This game wont' be uncensored.

I'm experiencing the same issue... I've manually edited the "Start in" and "Target" to the installation folder, but can you clarify what you mean on the last step? What link should I double click and what do you mean by it'll auto start it?

It's the lnk file you edited in point 3 (Ntraholic (Patch and Run))
As the name suggests, using that link will patch the game and run it (auto-start); after using it once for the patching, all future times you should use the .exe file of the game

Copied from translation text:
Default language will be Japanese but if you want to change to the other above mentioned ones.

you need to open the Ntraholic folder, then open the AutoTranslator folder inside. Then open the config.txt inside.
Be very careful to this config.txt. Look at the [General] and only change the Language=(Dont change FromLanguage=!!!)

Chinese:
Language=zh-CN.

Korean:
Language=ko

Japanese:
Language=ja

English(leave the right hand side of the "=" to be empty:
Lanuage=

After you change the txt, close the game and relaunch it.
