* Otherwise she will keep the dress if you complimented her looks in a dress, don't push for hot clothing and she didn't get together with Perry (Perry likes her old look so he influences her. Fucking Perry)
* Otherwise she goes back to her regular appearance.

I wonder why she went back to her old look for me then, but to be honest the whole mess was buggy as hell for me. In my playthrough Ian and her hadn't had a thing yet, though I did have Ian flirt with her as much as he could at the beach. I then randomly got a text from her about the bathroom. Outside of the bathroom she was in her old clothes. In the bathroom, which was hers and Ian's first time having sex in this playthrough, she was clearly wearing her hot getup.

I'll probably just replay the whole game leading up to the next Lena update anyway, a clean run.

Yeah, between all revisions for Ch.12 and 13 there were so many under-the-hood fixes and tweaks, it's probably best to have a clean save that goes through those parts, at least. Otherwise it may keep some legacy state that messes things up for the new script.
