One thing that may be holding you up is Jeremy needs to be angry with Ian, so make sure the relationship isn't very good by the point Jeremy tells Ian about Lena getting around, and choose the angry option, this will tank the relationship and they will not longer be friends. The other thing I think you Must do is have Lena take drugs at every opportunity at the mansion. If all is good you get a menu where you can choose Mike, Jeremy or both with a special icon that could mean "I'm really high".
