Ye it's not really, truth is I was so desperate to put some variety and this is one of the higher quality 2D (non-anime-style) graphics games.
Have Lena cheat on Ian, either with Axel or with someone at the club where Ivy works.
After Jeremy tells Ian about it, choose to confront Lena during the outing at Perry's beach house.
Don't break Ian and Lena up, choose one of the options which continue the relationship.
You should get option to Punish Lena after they both return to the beach house.
(also just guessing since i don't have script at hand, but Ian probably shouldn't be on the cuck path)

I'm very inexperienced when it comes to relationships but hugely vanilla and monogamous. It is hot to explore these relationship dynamics in game. But I don't know about real life. Maybe I might like it maybe not. But from what I've seen, these kind of relationships can get messy real fast in real life.

If you look beyond the scene in Chapter 13, Lena Goes On a Date with everyone except Ian, then her internal monologue hints at the future scene between Lena Ian and Axel. This is just my assumption, especially since when you play as Ian, there are relationship points with Axel — I don’t think that’s for nothing.
I suspect that, much like Ian's relationship with Gillian, if Lena remains hung up on Axel and especially if she fucks him again, this blocks establishing long-term relationship with other people. Lena's monologue if she fucked Axel tends to be along the lines that Ian doesn't really give her what she wants, and she doesn't see this relationship working out.

It's like this with Axel and Seymour. Lena thinks about breaking up with Ian if she is seeing anyone of them.
I meant that the condition is that in Chapter 12, Ian punishes Lena and she has a scene with Axel, and in Chapter 13, Lena goes on a date with everyone except Ian. It seemed to me that she was implying she needs both Ian, who punished her, and Axel, who she can’t forget. That’s why I came to that conclusion — that in the future, she’ll want both of these guys at once. Maybe I’m wrong.

If the shit you saw in games worked as flawlessly IRL, it wouldn't be a fantasy you indulge in by playing a game in your damp dark room away from prying eyes.
Man, it's even worse than your suspicions on all accounts:
- The game has VERY little content, except very mediocre scenes on the slut path.
- The cheating path was supposed to be only one path, yet it got most content, while the stag/Vixen path has had absolutely none, only talks and talks and "would you be open to inviting a guy", twice, thrice, then " we could give it a try" to FMF, and.....Nothing.
The worst part of the game and Eva Kiss' fucked up logic comes now:
- Technically, you can already have had one FMF three some Ian/Lena (with Louise), and two others lined up with Ivy and Emma HOWEVER:

Eva Kiss obliges your Lena to cheat on Ian with each girl before setting the threesome up.
