As a rule of thumb, taking drugs may impact the choices available to you.
If your Lena isn't taking Stalkfap seriously (i.e. only artistic nudes), has a relatively low Passion (below 8, I think) and doesn't take the drugs on both days, the whole chapter is just a normal vacation for her. It's pretty easy to not get involved in naughty activities. Even that Anthony guy is actually pretty aware of boundaries and kinda respectful which was quite a shock ngl, as an example you can have a mutual masturbation session and he doesn't even try to touch you if you don't (or can't) egg him on.
Wait, what? I haven't even played my Lena all that chaste (she's had her things with Axel and Ivy, both behind Ian's back) and she didn't do any sort of a gangbang, much less one without any input.

I want to access Ian's cuckold route (please no spoilers, lol), but I'm not sure how. Logically, I think he has to be in a committed relationship and not have an open relationship and still not have sex with her.

I was trying to make it a BBC (Best Friends with Benefits) route and start the cuckold thing, but apparently, if you go that route, he rejects you, and at best, you end up as friends with benefits. As it currently stands, Lena won't commit to a relationship with Ian if she's fucked Jeremy on the first opportunity available ( when louise goes out and leaves him in her room), you do get other chances later, if I'm not mistaken.
At that point in the game Lena's just not that messed up in the head, it happens slowly as you commit to the route.
The cuck route is pretty simple, reject sex with Ian but still commit to a relationship with him, sleep around for the entire game, and as Ian look at Gillian's pictures at the start of the game and make a pretty obvious choice when he dreams of her later on, "move on" on ch12. When Lena's giving handjobs and such ask about her experiences with other people. Strictly speaking you actually can have sex with Ian and start rejecting him later and still be on the route (just not the "pure cuck" route), I think the trigger is actually Ian asking for her experiences and a choice after meeting Gillian.
All in all it's one of the less enjoyable routes, I've found, to me it's pretty boring just going through the motions as Ian on it. Beats Robert's though.

Is Cindy realistic ? By all means yes, realism is probably one of the most admirable traits in EK's work. But it does not make her interesting.

I have, but they are nothing like Lena in the early chapters. The corruption path rings false in this game. For all females actually, Alison, Lena and Holly, all their corruption paths are poorly executed, especially compared to the other paths.

Alison - Fertility window is closing soon, and she needs a stable partner ASAP. If Ian has not shown any interest from the beginning, then she goes to full peacocking mode to lure him—no condom talks in the side chick path, dying her hair and getting piercings.
Holly - Went from casually dating a guy from the club to GH. Seems very fast, but you have to take into account that she was drugged by that skank Ivy. She was also being coerced subtly by the girls into that "independent" lifestyle. Having no friends to having literal 10s around you who acknowledge you makes you fall for the "Hero's Worship" trap.
Lena - This version of Lena falls under the dumb, money-hungry, broken type of girl. IDK man, she sounds pretty stupid to me anyway in that path, and it's my least played.
