What time frame? I'm on 1.06 if that makes any difference.
I've gotten all the girls pregnant at least once with each of the guys and have entered and left the hospital repeatedly to try to spawn the scene, nothing.

They might require another talk right after the first time they mention how big they have gotten.

Am I the only one who can't go any other places other than the beach and the inn? I'm playing the 1.07 version and always when i try to go to another place it's just says: "Go to the beach"
At some point a girl approaches you at beach for me it was in evening if I recall correctly then talk to her in Inn and should unlock other locations

Iirc..you supposed to talk all the heroine girls first before you trigger the blonde girl to come up to you.

late afternoon/early evening, you'll find her at side of massage table, you need to repeat the action with her till trigger cowgirl scene, then after that you can fuck her in the bath in the mid/late evening.
You can interact with her when she is:
1. In the side of the bathroom (you have to pay her and fight her then repeat a couple ot times)
2. Inside the bathroom water (still you need to fight her)
3. In her room at night (you need to complete 1 and 2 to unlock this one i think)
