No, you can turn it off it has no impact. Like folks here say you won't encounter NTR unless it is what you want to see.
Afaik ntr has an impact in that it is purely bad ends. That is to say if you choose the choice that leads to ntr, you get a portion of a scene, and then it ropes you back to the defining moment to choose the right option to continue the game.
There are the What If's, which initially started out as playing a ntr bad moment to it's extreme, including a full bad ending, but has since evolved to full on non-cannon stories. These are separate downloads.
So if you don't like ntr, always know it can be disabled and is a choice, and the content surrounding all the girls as the game has built has been proprotionally spread so you get alot of MC only action with the ladies. If you like NTR content, then bonus you have the base game and the what if's to enjoy.

They will probably wait for a few days for the full release as this is only Chapter 11 (it doesn't contain Chapters 1-10).
