Imo, the patch is possible because the creator knowing/unknowingly, conscious or unconsciously makes those forbidden relationship quite canon. Or the very least that relationship easily altered/pretty tweak-able. The 'landlady/roommate', 'guardian' you name it. The I-patch never been this big issues before, it was the same with other taboo or questionable in-game content, whether it contains r*pe or non-consensual content. afaik, they draw the line on M*nor/C*P* content only. But last year alone they change their TOS and demanding creator to explicitly saying every actions in-game must be consensual.
But they sure have some issues with their 'unwritten TOS', subtle censorship and content moderation on their platforms while pretending to be innocent.
The 'written TOS' mentioned shit about community modding, or acting like they never did any 'restriction of creativity' for that matter. But they're not as clueless or ignorant as they might shown either.
Well, you could easily guess they've got some pressure from the pro-censorship activist, and social movement is not an issue that business could ignore entirely.
They might not suspend your account, but also never said
they won't de-list or shadow ban your games from their platforms. So the AVN creators doesn't have much choice here. We know at least many if not most creators heavily depend on services like patreon/itch/subsc to gain financial benefit for their creations. Not much choice here, either you comply or completely stop using their platform.
