The guy is forced to take a pill that turns him into a girl for a day, to pay of his dept. Next day you turn into a guy again and can take the pill again or choose to stay male for this day.
At least that how it was on an older version, don't know if that changed
Also there was no fucking when I last played it, you model some clothes as a girl and can masturbate
