now that you chose a city by clickin on it, next you choose a girl by clicking on it. if then there are no boxes locked or unlocked to view its because that girl hasnt had a lewd scene yet.

The missing one from Springhaven is Mary. You unlock her initially by turning down Sarah's offer of a lift right at the beginning. After you do that, you hitch a ride with Bill in his truck instead. Bill invites you to sleep at his place for the night and you meet his Granddaughter, Mary. We haven't had the opportunity yet, but dev says we will still meet Mary later on if we chose the Sarah option at the start (and likewise, if we chose the Mary option at the start, we'll still meet Sarah later, as well).
The Lakeshore one hasn't been unlocked yet (dev spoke about this one after the last update, IIRC). General consensus is that you're right and it will be Melissa. They're in alphabetical order, so it's someone with a name coming between Dawn and Ruth, which fits in with that assumption.
Not quite - it means you haven't "met" the girl yet, to unlock her profile. There are several who show up and you can click on, but who don't yet have lewd scenes.

hotdamn that scene with millie and mommy. watched it in the replay, always miss all the good action in my playthroughs, a bit too obscure to get it right imo

I don't think it was in the slightest hard or obscure to get to. If you're actively pursuing both characters, it should just come up naturally, but you have to be on both of their paths, otherwise of course it's not going to give you the scene that involves both of them. Basically, do all the stuff with both characters, and you're golden - there's even an in-built walkthrough that tells you which are the "optimal" choices.

Just to confirm - Yeah, it is Melissa.
Obviously, you won't get the first scene you listed if Violet's path is closed - so yes, it's locked in that sense, but how could it not be?? ANY group scene will be locked if the other participant's path is closed. Sometimes an alternative scene will be shown, other times not.
Everything else that happened with Millie in this update can happen without Violet's path being open.
The other names you mentioned aren't locked behind other characters, although I can see why you think this at the moment; it's just going to take a little longer to progress with them if the other characters' paths are closed.

But I think I'll stick with fulfilling the mission for now, unless the dev has plans to bring back Miyuka down the line, always good to have another lady in the mix, with a chance at a threesome and we can always deal James later

Right now I have 2 paths going
first is my main one, where I accept Sarah's offer, and complete the mission, and kill the abusive husband
second is the where I reject Sarah's offer, fail the mission and not kill the abusive husband

I had about 30 different saves at one point, all with different choices chosen, so I could test updates with every scenario
It was a pain in the ass, so I eventually created a script that runs during the game, where I can just change decisions made, open/close character paths, etc, for testing purposes.
That's the pain for having so many branching paths, would only grow more and more too with the nature of how this game is unfolded

I did those two paths the other way round* (killing him on my Mary save after not doing so on my Sarah save). I'm wondering if there will be any negative outcome to killing him though, as there's certainly a positive one at the moment (preventing the scumbag from committing future abuse not even counted). I note that it's one of the few "this or that" options that do not have a green highlight, so it's totally down to player choice, rather than following the dev's suggested choice.
*Edit: Only the other way round regarding the killing/not killing, since I completed the mission in both saves.

Yeah, more work and headaches for me, but I think (hope) a better overall experience for players.
Yeah, whilst these choices can matter in the context of the game, there isn't a right or wrong answer, so I leave it to the player to decide what they want to do.
There could be consequences for each choice made/not made, but it's never wrong; the story just evolves with your decisions.
Some of the more mundane choices can impact the game without you even realising, e.g., in Rosie's cafe, you have a choice to tell Melanie you'd like to help her or not - this is where you decide the fate of asshole1 and 2.
