A questionnaire asking question of some key decisions (relationship with the twins, is X corrupt?, who you decided to side with?. etc) may not be a perfect solution, but is easy to implement and shouldn't impact significantly the playthrough so is workable. That said, it works but considering the amount of decisions that you can make in a run it should be an option for those who are not migrating from* Act I or want to skip to Act II and are not too worried about detailsA questionnaire isn't a particularly good solution. Just think of how in depth it would have to be to actually capture things adequately. I'll use one of my runs as an example... this particular Rowan avoids cheating on Alexia in virtually every circumstance that doesn't involve a female Goblin. He bangs basically every female Goblin that offers, but touches no other woman except for Alexia. Alexia, on the other hand, is a giant ho bag who fucks everyone and everything that offers. Simple in concept, but unless there's a question that wraps that up that's still at least half a dozen choices just on female goblins without factoring in every other sexual partner for Rowan, and a boatload of variables for Alexia. The only way to easily account for everything is save carry over, which is fully possible between stand alone games in Ren'Py. Of course the problem there is building a story that accounts for any and all choices.
My suggestion would be to handle it on a case by case basis. Instead of starting with a list of questions you introduce questions as they become relevant. So at the start of the game you get asked about the broad strokes. What was your reason for entering the Twins' service, are you involved with one of them, your relationship with Alexia, and Alexia's relationship with the twins, and finally who you aligned with in the assault on the capital. Then you go to visit Cla-Min and it prompts you with questions related to her, and you repeat this process any time a possible carryover event is relevant. This would allow the player to be eased into things instead of just vomitting questions at them. It would also help with remembering your choices since you're interacting with the relevant characters.
Can't get into details bc I don't know how is implemented but this ain't gonna be the first game that will offer an option to migrate a save file between different Acts or Chapters, but should be doable without resorting to black magic
