I mean the various character infos already confirmed which characters are targeting which fetishes. I believe Iris, Anitta, and the brothel owner will focus on feminization and/or crossdressing to varying degrees and from varying angles. But the other characters won’t focus on that or probably won’t feature it all.

Some will apparently do it if Spacer likes it. I believe Tessa is one of them but it will be more of an active choice

No, but almost all of them are unlocked by simply completing the story, unlocking sex scenes, or some other minor details (like earning cat's named Captain trust in Lisa's arc or feeding Ginny's pet). Either load your saves if you made any before the choices and forks, or simply replay the game, choosing different options and skipping everything.

At first glance, you might think so, but VP does his best not to cross the line and make himself look ridiculous. MC isn't able to handle the larger sizes at first either; it takes training to better absorb each size, and even then, I don't think he'll be able to handle them all.
