As for the pregnancy question - only the developer can answer it. I personally do not care about this issue.
Whether Aunt Kate gets pregnant or not depends not on our desire, but on the developer's desire.

It's up to you, but a number of scenes are actually changed at the beginning of the story. They don't seem to break the game if you use the old saves, but I would personally start the game from scratch.

No, you invite aunt to stay with your step family.
The rest should be fairly easy to do.

That took quite a while for me too, but in essence, you need to have:
- creampied both (preferrably analled Jill as well from memory...)
- had group sex with both (with other fam members etc.)
- stay with Jill in the same room
- invite aunt to stay in alternative room
- wait for Jill to come back from work IN THE CARPARK*, any night that she's working will do. Just make sure you wait from 2AM onwards.

Hmmm it's been a little while but... do you remember if you joined your step fam for dinner enough times? I believe the conversation needs to come up first.
THEN you talk to the aunt about the same... it should then open the option up for you.
At least that worked for me!
Edit: pls ignore this as CbunnySwtor has the best guide - I totally forgot about the Lily bit. But if you said that you've triggered *all* other scenes, which should also include a Mary/Lily scene, then this must've already happened for you, no?
