I unzipped the main file then copied the incest files, from the incest patch, into the "game" folder. When you start the game it should now say it is the "family" edition instead of the "commune" edition.

It over writes the relationship names used in the text. "Holy Mother" becomes "Mom", "postulant" becomes "Aunt", "Abbess" becomes "grandmother", etc...
