Without an actual incest patch the writing won't make sense just like every other game that allows you to set relationships like this one. So really it isn't written with incest in mind because the dev can't be bothered to create an actual patch.

Issue with most of these type of games. They cram the words brother, sister, mother, father down your throat. All you really need to do is establish relationship. Rather than use formal, use informal, pet names etc. Using Bro, Sis, mom, dad, etc work as well. It's the story that make it good, not how many times you can use the word sister etc

Idk about the " Sister" part, but Hulk Hogan would call everyone "Brother", even the women.
