I got it now, when I played intro. Vlad Dracul (Vlad the Draagon. He was a member of Order of Dragon, that's how he got nickname).
I'm not sure if 3 counts as harem but there will be other characters

Regarding your question, some relationships will be optional but most of them will be unavoidable.

If this was my current playthrough of Crusader Kings 3, He'd just marry his sister and take his daughters as concubines. And none of them wear clothes because religion (started out as Adamites, thanks randomised religion start).
Also, not a comment on this game, but the (casual) history nerd in me can't help but notice that the bathtub he's using is a few hundred years too early, the round shields on the walls are a few hundred years old/too late, and he must be an ultra-minimalist (or really poor), since he's got no render (or even just limewash) on the walls, let alone simple murals painted on them. It is a porn game made with what I assume are Daz assets, so it's not really an issue.

Dialogue feels like random and emtionless. Like some robots are jumping line from line. King is a spineless idiot who gets pushed around. How did he even became a king? Not any choice so far.

The King seems like the worst kind of douchebag: A 'bicycle rider' - kicking those below him, while bowing like a spineless worm to those he can not quiet reach with his feet. Really hope we can influence that with choices later...
Imo it would be nice if he showed some strength and cunning before any potential 'magical' interference happens - outside of 'ordinary school boy gone chosen one'-settings, it's always cooler if a harem is 'earned' in some way, rather than being just granted by some supernatural mind bending force / godlike intervention and stuff like this. Else it always has that bitter taste of being just an impotent power-fantasy.
Still an interesting setting and much potential for the story to grow nicely. Neat models too, especially dig the unique and fitting look of the king. The (planned) tags will likely also 'speak' to enough people to get some interest going.
