Haven't been there yet in this game, but presumably it works similar to his prior game Tapioka-Chan, where just fingering those spots isn't enough, you need to apply the special drinks that lubricate, cool and ease pain, raise her lust or drunken state to boost her pain tolerance... With Tapioka it was very easy in the onsen bath (lubricating...) so maybe use the bathtub to your advantage?

I just wish the save was like yotogimichi.

i did it and i didn't have any of those things.. just play with it a lot..
the last game was like that too, i just played a lot and keep on trying to force it in.. eventually it worked, and funny part is that right after i got it to go in like i wanted, the expanding items unlocked.. oh well, funny, but i didn't need them at that point

Version 1.1.0 came out on May 8th
QoL improvement that adds a body parts menu.
Max inventory toggle.
Minor bug fixes.
etc...
Seems like more patches are planned in the future.
Bousoft last game Dereah Sister Hypnosis App was released on 05/26/2023 (V 1.0.0), and the dev actively patch the game until 01/12/24 (V1.2.8).
Looking forward to dev continuing to work on the game and maybe a DLC?

Deepthroating her with your dick on auto mode (either in normal or possession stances) will gradually increase sensitivity. Just don't use click or double-click actions, otherwise sensitivity will decrease.
