There is no other world set in the stage... "AI Translation", "DeveloperMTL", "EMTL", or what ever else... MTL is MTL.

Why is it mtl if there is an english dlsite version of it?

Based on the description on dlsite it seems like the english version is the AI translation. Kinda misleading, the preview pictures seem to be manually translated but I'm guessing the actual game will resemble the preview pictures on the thread here.

EDIT: Based on reading the translation request thread both translations are basically wordwrapped MTL, so expect poetry, MuninKai was part way through cleaning up the MTL when the 'official' release was announced so they stopped, though they have said they might revisit it now it's clear it's another MTL.

already played thru the game with an early mtl from here... this game has a lot of wasted potential for the preg content, especially considering the previous game, Key of Egg. pregnancy stuff only appears in 2 missions, one where it's for a special dungeon/mission (will have a special icon on the select screen) and the other is a loss condition if ur corruption is over 100 at a certain point (I don't remember when exactly)
