It adds in-line images to certain scenes. These images can be expanded by clicking on them if you want a larger view.
For example, during the Flahne underdesk scene you would see the left attachment with her ovipositor hidden, and the right attachment with her ovipositor out:
There is also a new button on the character image pane (third from the top) which lets you expand those images as well.
The image pack .swf contains around 470mb of extra data compared to the filesize of the normal version. However, don't expect every sex scene and scene variation to have appropriate in-line images added, it's very far from being that comprehensive.

Something I noticed a while ago... the Wiki says that Xil (bee girls) can get pregnant by a male MC and... that's it. No details on how or if they show up again, no mention of what becomes of the offspring, no follow up of any kind in fact.

You can get a zil female pregnant. After that there is a repeatable encounter of the pregnant zil during which you can talk to her, suckle her breast, take a nap or have sex with her. No interaction with her offspring that I can think of.

Same but I'm more annoyed about the save editor can't create the perfect sex avatar when I have to spend an hour in the wiki looking through all the transformations and then playing russian roulette to get the specific body part, the artwork is not that fancy (the vaginas are really drawn badly) for me to care about it.
Sex scenes are text (convenient for me since I'm a huge fan of erotic stories/ sex stories) and they tried to keep PC's side as open as possible so the notion of having "perfect" avatar seems a bit obsessive because anything remotely close to what you have in mind will be portrayed same in text.
However already both in-game or with save editor you can change EVERYTHING about your avatar. Don't exactly know what you're looking for which is not already available. Point it out maybe we can help.
As for artwork there was a lot a debate about it in forums. It seems the artist wanted them to look this way to give a feel of "alien" species which should be of no importance since fenoxo's games are most focused about dickgirls/futas and delivering that sweet but rare degenerate fetish of a 16 inch girl dick pounding a femboy. If you want vanilla focused sex then you're in the wrong place mate!
