The girl "helping him" is a sister figure, not a sister and netorare only refers to the MC being cheated on, which doesn't happen in this game. The title is both incorrect in the case of NTR and mistranslated in the case of sister being used.

I'm with you, I personally enjoy a game with consequences and risks, but then you always find either the NTR fans being very critical when someone asks if it's avoidable and the harem fans that will always get angry when a game has both the harem and NTR tags.
