Is it possible to "modify" the character's appearance after the game begins? That is, can she undergo plastic surgery, change her face, etc.? Or is this only possible during character creation at the beginning?
Okay, played this game after 1 month pause. I noticed that my Settings stayed in place... Looks like no more need to adjust them each time I exit game and open it again. I did my usual routine... Check, check, check. Somehow, kind of bored of doing the same stuff for 1500th time... Everything is easy for me by now and I am glad for it, I hate when games frustrate me with something difficult. But, it's always the same program. The only change: the snow, christmas lights... There is sex... The sex isn't good. No change there. Too bad. I played my Slum Jane save, so no pizza cuni for me, unless I could order pizza to the bunker and Ralph would do it for me? He is kinda my roommate;-) But, I guess I'd only see this:

Every NPC reacts the same as always to me and I can't do anything to change any relationship dynamics.

Afaik/r from your posts, as well as the general tone from the game: The sandbox freedom, options to design your character, details like there is x-ray (yeah, without cum), but most of all the "forced" options, ?that will get toned down? in new game. Then you thought about the things you don't like and the length of content.
That's likely the games strongest problem and partly being the fault of how it was built. The new version shall fix that. I do hope some things like the mini-games will be toggle-able, at least you can see the vision so one can only hope it works out. Afair he also hired someone for writing so there should come also more on that side.

But, if by the "new version" you mean the Revisited, then no. Apart from my utter uninterest in mini-games of any kind, the overly complicated and demanding life sim mechanics, and my very favourite dub-con/non-con being "toned down" (yeah, please try tone down a rape ) , the Revisited is completely different category. I don't even remotely like how the people look like. How they walk. I won't even download that one.
- yes CE and the table are open source to know its safe to use, but be aware of the installer with ads in the free version, see spoiler below). Its a well-known tool since 2000. In a nutshell: Get/start tool, load table/or attach and auto-load, hit enable and activate the wanted options.
Generally keeping saves before/after is advised. WARNING: you can break game events if you set items or stats you f.e. are yet not supposed to have!
v7: you can choose to either open a structure form to edit items there (for pure edit may easier to look at) or add them to the table (because only there you can freeze f.e.). Note: You can also add Items from structure to table individually (select those you want to add and hit "A" key). See second pic for usecase. Also no more stuff to do ingame, to get values!
