I have 16GB of DDR4 RAM and the game works fine in High graphics, RAM usage is below 9GB in the city.

The error you're mentioning is about the GPU (D3D Device = GPU).
Generally, when you have an error "D3D Device is lost", that means your graphics driver crashed.
Maybe try this :

2 - Set the virtual memory/paging file to
"at least" the same amount of RAM you have (in your case 16384 in min and max fields).
What it does; simply speaking : The Paging file and your RAM works together, the paging file serves as a cache to help your RAM.
Per default, values of this cache are actually random since Windows does it in your back and that can cause a lot of issues in any games.
