Just like in Windows and Linux, a Ren'Py game on a Mac has no need to be installed and doesn't come with an installer. Just extract the zip archive and there's a WiAB-Chapter4 application "file" in the main folder of the game to start it.
Did you extract the zip or just looked inside it? Inside a zip it may look like a folder with an *.app extension. It's because it is an actual folder, Mac simply recognizes it as an application, but maybe not while it's still inside an archive.
In Windows and Linux it still looks like a folder after extraction.

The only thing we will get from Ocean in the next year or year and a half for this game is photos because he is focused on Summer's Gone until Season 2 Ch 1 is out then I see a long break to recharge.

Is there any actual reason, why he basically rewrote all the relationships? Kinda takes the whole spirit from the chapters before and basically makes the story kinda useless and takes the fun. Like why would some random care this much about randoms who aint his family? Was such a great game, even after the first rework and always was thrilled for an update. but now it doesnt even care anymore. sorry, if he wants to change the whole game, just make a new one besides this.

Might have something to do with the Patreon for this game getting nuked in the past and steam and itch cracking down on nsfw content due to pressure from banks and payment processors.
But even with the rewriting its still nsfw? And Im actually no expert in this topic, but all the other games get it done too, aint they?

The crackdown on nsfw has been mostly focused on rape, non-consensual content, and...incest. WiaB had incest.
Ocean decided to future proof his game (at the expense of some of his audience complaining) by removing the incest so it can stay on sites such as Steam.
Why wouldnt I be serious? There are a lot of games still with these genres and they also generate money and also this game is obviously still nsfw so what about it should be a joke?

Because with the incest intact, Ocean risks Steam banning the game on their storefront and he will lose a chunk of income. Subscribestar also has a clause about incest in their TOS but they aren't big enough yet to warrant attention from payment processors so they don't really enforce it.
