It makes me wonder if there is some invisible piece of the UI loitering around on the screen, that puts things into that non aggressive mode, and pressing escape cancels it.

Yes, you are correct. The reason they don't attack is because they think you are still in the masturbating state. If you masturbate and attract monsters then hit your masturbate button again to stop it then it doesn't work. This has been a thing for many builds now. You can do this trick and walk anywhere and nothing will attack you except the Iron Creed. It seems they have different protocols when it comes to combat. For example, if you have a monster servant with you and try to get it to attack the Iron Creed it will not attack it. Night time enemies are similar and will not attack, tho I think some will idr which does, but most seem to not be able to.

So you still have to run past the Iron Creed and also if you go into a dungeon it will reset or of you save/load it resets.

You can also get in a masturbation right in front of monsters without this glitch if you do it quickly after giving birth. There is a brief window that allows for it, but if you have the staff then it kinda becomes pointless.

I think if understand that video that you simply fell through the platform.

Either the F key or cheat menu to hit minus key moves you out of being trapped.

(F key for the player menu which has an option to get unstuck)

The fall through a built platform level on load is unfortunately a long-standing bug - something on their load logic is wrong. But it is a stable and predictable bug...

Oh there also i enter the dungeon left of fisherman hut and i tryin to fight monster there. they jumping over my head push me stuck under the floor and making my fight already hard as it is too
yup guess that all i gonna play for now
maybe couples more years like many forever in early access game around here i come back to check later with how unstable each update i must start over it is

spent a while playing this yesterday, got some decent equipment and levels, but when I went through the protal to get to the woman who wants you to get knocked up by a beastman, inventory was wiped and char was reset to level 0...

anyone else, just me? is there a fix/workaround?

Which versoin are you playing? This used to happen with old versions but I have not seen it myself in any of the Steam versions nor since 8.6.1

Maybe this will help, maybe not:
Put the patch updater into the same folder as the game EXE file. It probably doesn't need to, but it won't hurt anything to put it in the same location as the game files.
The Installation Path in the updater should match the path to the game files.
If your game is buried deep in a folder tree, you might want to consider moving it to a top level or just a few folder levels closer to the root of the drive. It may not make a difference, but I know some programs can have issues with long file paths.
If you have the Installation path set correctly in the updater, it can take a minute or so to detect the version. I think you need to be on version 20424231 for the patch to work.

I tested (this is Steam on Steam) current and now the Servants do indeed attack Iron Creed. It seems one can't order them to attack but they now organically attack and/or defend themselves. Unlike previously where the Servants not only couldn't be ordered to attack Iron Creed, they wouldn't and would even just stand their ignoring getitng massacred.... my test was with the Iron Creed batch near the desert waystone, and not only did they not ignore, they won the battle (to an extent - 2 survived and no Iron Creed did). At least that annoyance is fixed.

Re chests, specific keys are needed.

I'm using the 20424231 version

/edit:
deleted (hopefully) everything, reinstalled, now I can get into the tower, but not the fallen king tomb...
