When I start the game and go to gallery, every scene is already unlocked. Is this how it is supposed to be/ how can you tell which scenes you have seen ingame?

Can't say for sure why, maybe the download here is already with fully unlocked gallery, but in the original game, you start out with a clean one. Unlocking gallery along with main quest progression was the original idea

I think he unlock is for the complete game, NOT for each playthrough. I believe if you delete the save folder, it will restart fresh.

it’s a permissions thing. You could either go through and Chmod a bunch of things. Otherwise it’s much much easier to download the windows version and port it across. I have a guide here, alternatively you could try m4kro’s rpgm launcher which is probably even more effective

There's permissions, as Ungaa mentioned, which you can fix with chmod, but there's also quarantine imposed by Gatekeeper. The latter will prevent you from saving your progress, even if you can get it to run.
Removing quarantine in Terminal: xattr -d com.apple.quarantine (path to nwjs app)

There's already a "monster girl" tag, why the f does monster girl games also need the "monster" tag? It's confusing af, now I don't know if the game has monster girl AND monster cock or if it's just monster girl.

It has both monster girls, and "normal" monsters (horny plants, gnolls, etc). Monster girls all serve as bosses, and later move to your "base".
