I think the tag is only used if MC gets someone pregnant (or IS pregnant for a female protag) or has sex with a pregnant character, not just because someone in the game who isn't a LI happens to be pregnant...

I've seen it used for pregnant sex as well.

Again, the tag system is arse for nuance, especially once you consider that we have very specific scenarios that have very specific terms that are represented by mixture of more superficial terms that are used as tags.

The most egregious example being cheating scenarios, for which we have like four words but two tags, all of which may cross over with each other.
