you should get the 4th heart during this update while sex scene between Val and Amanda. If you got this heart and still have just 3 you missed one before - no clue where this could be

download the new version.. copy your previous saves from www folder to your new version of the game's www folder.. and play

but i dont know if its possible to just copy one save file... maybe need also the global save and config save? never tried out a foreign save file in this game

The game does not count coins and karmasutra pages towards completion of a version. It only counts scenes. So you can still go searching. (I know, I miss two coins and two karmasutra pages myself but heck, I am too lazy to search every single place again ;-) )

OR just use one of the save games ppl are kind enough to offer in this thread.

for those who need savegames:
this is my savegame folder - having several savepoits between 38% and now 54% 100% content (easy mode)
just copy this one into your WWW folder and choose the point you want to start with (or 54% to jump to end of current update content)
