Choices have changed a bit, so you might have brought yourself on a wrong path and locked yourself out of content of Carol if also for example on love path for Amy. Think the solution for that has been posted a few pages ago.
As I recall, Lilly is a hermaphrodite (I use this term to refer to a human being (not a demon, etc.) who was born that way and not changed into this by magic or breast implants). Lilly thinks of herself as female but has a genital malformation (congenital adrenal hyperplasia) that makes her repugnant (or she thinks so anyway) to men to whom she would be attracted.
