As far as I can tell - nothing. I got the crystal, the Elf Charm from the Forest, the sword and magic rod from the castle and a bunch of other items, but nothing really happened.
The only item you really need is the Key for the Monster Institute. You get it after talking to the boy near the far right door on the first floor, then going to the second floor and interacting with the mustached NPC on the table. This will allow you to unlock the door and have fun with the monster girl on the other side.
The only real "quest" you have in this game is talking to the King in the castle. He will tell you that there is a demon queen, then you can exit the castle and exit the city by either going right or down. This will make you travel to the demon queen's castle and voila - the game is complete. Behind the queen's throne is the CG room, so any girl you've missed will be there.
i think its more a case of him actually supporting his own narative of it being an actual game and you are breaking the rules by fucking everyone

of the game there is one thing that I don't understand... why there are items if they are totally useless I would understand if there is some kind of secret with other chara or that you unlock other scenes for already present chara but you just pick them and nothing else
This game gave me a headache, i know its impossible for this to ever happen but the MC acting completely cool with being trapped in a reality where nobody can talk to you is terrifying.

Yeah, this NPC game rates at the top for the arts... though there being a lot of those “unusable” items felt like the game should have been a lot more than it currently is, that would have been awesome. But then again, not “removing” them can make one ”haunted” with the feeling of “is there more?”, ”are there secrets in this game unavailable in the gallery?“ or something like that.

in the demon castle, behind the demon queen there is a door, if you interact with it then it warn you that behind it there is a room with all the scenes and you can enter if you don't care to explore anymore(Can’t remember if this one has a gallery, teehee..
