From looking behind the curtain of the game I think they just effect the image that hows for your Lewd Goggles screen. The H status ups are tied to the events completely and having the tattoo vs not having it doesn't do anything mechanically that I saw.
So it will happen regardless of what you do in the game? or is this fight only occured with a certain route.

The lewd jobs? (never really found, ryo was either one shotting or i wasn't fighting bosses for scenes) They probably appear after you hit a certain point of lewdness, lewd event/battles, or during ntr route. I do know Ryu gets tp skills depending on what he does at night(aside from the random lira ripoffs around town, brothels actually gives him skills->example : the infamous "Julia" gives you one at pope town) and whether you are dating lisa or lena.

The lewd underwear? I do know if you hit tender/like,lewd, mature etc.... with every stat (breast, vagina, mouth) the outfit changes. with the new translation lewd status mod posted earlier (shoutout to Silentce) it looks like this:
